Conventional medicine manages this two ways: ride it out with support, or transfer the dependence to a maintenance opioid like methadone or buprenorphine, which works and saves lives but keeps the receptor system occupied indefinitely. What conventional medicine has never had is a way to simply turn withdrawal off. That is the specific thing ibogaine appears to do, and it is why people who have failed every other approach get on planes.
What one treatment actually does
The clinical pattern was first formally documented in the 1990s: patients in active withdrawal given ibogaine showed resolution of withdrawal signs within hours, without opioid replacement.1 Providers have replicated that pattern thousands of times since, and it is the most consistently reported effect in the entire ibogaine literature.
Mechanistically, ibogaine and its metabolite noribogaine appear to reset opioid receptor systems toward their pre-dependence state rather than substituting for the drug, alongside a boost to GDNF, a growth factor that helps restore dopamine circuitry. The outcome data, honestly
There are no completed randomized controlled trials for ibogaine in opioid dependence yet. What exists are observational studies, and they point the same direction:

Read those honestly and two things are true at once. The withdrawal interruption is robust across every study, and the long-term picture is mixed: many people relapse, some within weeks, and the people who stay well overwhelmingly tend to be the ones with structured aftercare. These are self-selected patients who traveled abroad and paid out of pocket, so the numbers cannot be waved around as general success rates. The fentanyl era complicates everything
Most of the published data comes from the heroin and pill era. Today's street supply is dominated by fentanyl and its analogs, which are more potent, linger in body fat, and are increasingly cut with sedatives like xylazine. For ibogaine treatment this changes practical things: providers typically stabilize fentanyl patients on short-acting opioids for days to weeks before treatment, screening is even less optional, and protocols run more conservatively.
It also raises the stakes of the methadone and buprenorphine question. Long-acting maintenance opioids must be tapered and switched to short-acting opioids well before treatment, under supervision, or the treatment simply does not work properly and the risk profile worsens. Ibogaine substantially resets opioid tolerance. Someone who relapses at their old dose after treatment is taking an amount their body can no longer handle, and a meaningful share of post-ibogaine deaths are exactly this: overdoses in the weeks after a successful detox. If you take ibogaine and later slip, the old dose is now an overdose. Every treatment plan needs this said out loud, and a relapse plan built around it.
Why aftercare decides the result
Every clinician I interviewed for the course said a version of the same sentence: the treatment is the easy part. Ibogaine hands you a window, days to weeks of absent craving, lifted mood, and unusual mental clarity, driven by noribogaine and whatever deeper reset is happening underneath. Windows close. The people who do well are the ones who fill that window deliberately: integration support, therapy or coaching, a changed environment, moved contacts, structure, movement, and community.
The people who struggle are usually the ones who fly home to the same house, the same phone, and the same dealer, with no plan beyond feeling better. That is not a moral failing.
